Care Coordinator

We are seeking a compassionate and organized Care Coordinator to join our care home team. In this role, you will play a vital part in managing and coordinating the delivery of personalized care services to residents. You will serve as a key link between residents, their families, and the care team, ensuring that all aspects of care are tailored to meet individual needs in a supportive and nurturing environment.

Key Responsibilities

• Assess residents’ needs and develop coordinated care plans
• Act as the primary point of contact for residents and families regarding care services
• Schedule, organize, and monitor care delivery across interdisciplinary teams
• Ensure timely communication of changes in residents’ conditions to appropriate staff
• Support residents and families in understanding care options and resources
• Maintain accurate documentation and records of care coordination activities
• Ensure compliance with healthcare regulations, policies, and best practices

Qualifications

• Bachelor’s degree in Nursing, Social Work, or Healthcare Management (or equivalent experience)
• Previous experience in care coordination, case management, or a similar role preferred
• Strong organizational, communication, and interpersonal skills
• Ability to collaborate effectively with multidisciplinary teams
• Compassionate, patient-focused approach with attention to detail
